Sports Physicals for 2017-18 School Year

Location: Lapeer High School Cafeteria

Fee: $15 cash or check made out to “LHS Athletics”

For: All grades, all ages who need a physical. Non LCS students welcome!

Important Information:

* MHSAA physical forms will be provided on-site or may be downloaded and filled out in advance at: https://www.mhsaa.com/portals/0/Documents/health%20safety/physical2page.pdf

* Only this new MHSAA physical form will be accepted, please no older forms

* A parent/guardian must sign the physical form
