Time: 6:00 pm – 8:00 pm
Location: Lapeer High School Cafeteria – 933 S Saginaw St., Lapeer
Fee: $15 cash or check made out to “LHS Athletics”
For: All grades, all ages who need a sports physical for next school year
Important Information:
- MHSAA physical forms will be provided on-site or may be downloaded and filled out in advance at: http://www.mhsaa.com/Portals/0/Documents/AD%20Forms/physical%20card%202%20page.pdf
- A parent/guardian must sign the physical form
