Date: Monday, June 1st 2015
Time: 6:00 pm – 8:00 pm
Location: Lapeer High School Cafeteria, 933 S Saginaw St
Fee: $15 cash or check made out to “LHS Athletics” ($10 of each physical goes back to LHS Athletics)
For: All grades, all ages who need a sports physical for next school year
Important Information:
- MHSAA physical forms will be provided on-site or may be downloaded and filled out in advance HERE.
- A parent/guardian must sign the physical form
