Required sports physicals for student participants from Lapeer East High School, Lapeer West High School and Zemmer Junior High School for the 2009-2010 school year are being offered at the Lapeer Regional Physical Therapy Center at 1057 Suncrest Drive on Wednesday, August 5th at 5:30 p.m.
This is a new location from previous years.
The cost is $20.
In the event parents or guardians want to have the physicals done by another doctor they should be aware of the following requirements:
Sports physicals for the 2009-2010 school year must be dated on or after April 15, 2009 and must be recorded on the Lapeer Regional Physical Form.
No other physical form will be accepted, including the Michigan High School Athletic Association form. Parents not attending the August 5 session can pick up the forms at the office at Lapeer East and Lapeer West High Schools.
A parent or guardian must sign the physical form.
